维持性血液透析患者铁代谢水平对心脏结构及功能的影响

    Effect of iron metabolism on cardiac structure and function in maintenance hemodialysis patients

    • 摘要: 目的 分析维持性血液透析(MHD)患者铁代谢水平对心脏结构及功能的影响。方法 纳入2016年1月至12月在武汉大学中南医院行MHD治疗的患者91例,根据KDIGO指南中铁代谢靶目标,将患者分低铁代谢组、铁代谢达标组及高铁代谢组。观察各组患者基本情况、实验室指标及心脏彩超结果,并进行统计学比较。结果 3组中,低铁代谢组血红蛋白水平最低(P<0.05),铁代谢达标组贫血治疗达标率最高(P=0.001),余数据各组间无明显统计学差异。心脏彩超结果通过Logistic回归分析显示,高龄、高血磷是心脏瓣膜钙化(CVC)的独立危险因素。高龄、低血红蛋白及高铁蛋白是心脏左心室肥厚(LVH)的独立危险因素。而LVH的发生在低铁代谢情况下,与CVC、年龄、白蛋白有关;而在高铁代谢情况下,高铁蛋白是LVH的独立危险因素。铁代谢达标组LVH发生率(14.29%)最低(P=0.003)。高铁代谢组二尖瓣血流舒张早期最大流速/二尖瓣心房收缩期最大血流比最低(P=0.015),而3组间CVC发生率及左心室射血分数无明显统计学差异。结论 MHD患者铁代谢的达标情况与LVH的发生密切相关。无论是低铁代谢还是高铁代谢均对MHD患者LVH及心功能存在影响。

       

      Abstract: Objective To analyze the effect of iron metabolism on the changes in cardiac structure and function in maintenance hemodialysis (MHD) patients.Methods This study enrolled 91 MHD patients between Jan. 1, 2016 and Dec. 31, 2016 at Hemodialysis Center of Zhongnan Hospital of Wuhan University. They were recruited with general information, laboratory results and Doppler echocardiography. According to the Kidney Disease:Improving Global Outcomes (KDIGO) guidelines, the patients were divided into low iron metabolism group, target-iron metabolism group and high iron metabolism group. The general information, laboratory results and Doppler echocardiography results were observed, and the relationship between the target-iron status and the changes of cardiac structure in MHD patients was analyzed.Results Hemoglobin level was the lowest in low iron metabolism group (P<0.05), the target-hemoglobin rate was the highest in target-iron metabolism group (P=0.015), and there was no significant difference in the rest laboratory results among three groups. In Logistic regression analysis, Doppler echocardiography revealed that advanced age and high phosphorus were risk factors for valvular calcification (CVC); advanced age, low hemoglobin and high SF were risk factors for left ventricular hypertrophy (LVH). Moreover, CVC, age and Alb were correlated with the development of LVH under the low iron metabolism conditions, however, high SF was risk factor for LVH under the high iron metabolism conditions. The prevalence of LVH (14.29%) was lowest in target-iron metabolism group (P=0.003), the transmitral E/A ratio was lowest in high iron metabolism group (P=0.015), but the prevalence of CVC and the left ventricular ejection fraction (LVEF) showed no significant difference among three groups.Conclusions The target-iron metabolism rate was closely correlated with the development of LVH in MHD patients.Whether low iron metabolism or high iron metabolism,LVH and cardiac function will be changed in MHD.
